The Federal Mine Safety and Health Administration has concluded three safety failures by operators of the Metikki Coal Mountain View Mine were to blame for an accident that killed 25-year-old miner Joseph Mitchell. Mitchell was operating  equipment in the mine and was killed when he was struck by an out-of-control locomotive working to deliver supplies to the mine section. One other worker was pinned and injured. The official cause of the accident was three failings of company policy.
